What happens after I submit my New Patient Packet?
A practice representative will contact you to schedule your new patient appointment once your forms, photo ID, and insurance card(s) are submitted.
What should I bring to my appointment, and when should I arrive?
Please bring your insurance cards and driver’s license/photo ID, and arrive 30 minutes before your scheduled appointment to address any outstanding paperwork or insurance issues.
