Whitfield Family Practice provides annual physicals for the whole family at our Decatur, Alabama office inside the Med-Surg Clinic. An annual physical typically includes a review of your health history and current medications, a check of your vital signs such as blood pressure and weight, and a general wellness assessment. Because we treat patients from 9 months to 90 years of age, annual physicals are part of care at every stage of life — from a child’s well visit through an adult check-up or a Medicare Annual Wellness Visit. Our providers, Dr. Amber Whitfield, MD, Liz Berry, PA-C, and Chase Fortune, CRNP, along with our team of experienced nurses and office staff, work with each patient individually to provide the highest quality care and improve overall well-being.

This form is separate from our New Patient Forms, which are for patients who are new to Whitfield Family Practice. Once submitted, a member of our team will follow up to get your appointment scheduled.
